Creating And Editing Questionnaires

The purpose of this article is to provide you with a simple step-by-step process for creating a questionnaire in Activ8 software. We also explain how you can edit and archive a questionnaire.

Creating A Questionnaire

To get started, navigate to the ‘Questionnaire’ section on the left-hand side of Activ8.

After selecting ‘Questionnaire’ click on the +New Questionnaire’ button.

Give your questionnaire a name and a description to explain the contents or purpose of the questionnaire.

Then click on the ‘New Section’ button.

Doing so allows you to begin creating the content of your questionnaire.

Fill in the heading for the section and click ‘Save’.

Next, click the ‘Add Question’ button to begin adding the questions for your questionnaire.

Once you have added all the questions you require, click ‘Save Questionnaire’

Activ8 will save your questionnaire to the ‘Questionnaire’ tab.

Editing A Questionnaire

You can edit the saved questionnaire by going back to the ‘Questionnaires’ tab and selecting it.

After selecting the questionnaire, it appears on the right where you can make and save any changes.

To edit your question, click on the pencil icon.

Once you have made any changes, you can click on the ‘Save Questionnaire’ button.

Archiving A Questionnaire

To archive or delete a questionnaire, first navigate to the ‘Questionnaire’ tab.

Under the ‘+New Questionnaire’ button, you’ll find all your saved questionnaires.

If you no longer want to use the questionnaire, select the red bin icon as seen above.
